12/13/2023 EnchantaVaughn Approved QMHPA greeted Chevelle upon arrival to the home setting. Upon arrival to the home setting, Chevelle appeared to be in good mood evident by her overall disposition. QMHPA began the session by inquiring about Chevelle current thoughts, feelings and emotions. QMHPA listened to Chevelle discuss any new problems and/or concerns since last session. QMHPA probed Chevelle about feelings of anxiety and depression. QMHPA reiterated utilizing coping skills when negative feelings are triggered. QMHPA encouraged Chevelle to continue using her journal as a healthy outlet to verbalizing her thoughts when she's feeling overwhelmed and/or having difficulty with communicating her thoughts. QMHPA taught Chevelle relaxation techniques to learn to calm down when feeling stressed and having thoughts of negativity. QMHPA assisted Chevelle with identifying automatic negative thoughts that contributes to her negative attitude and hopeless feelings. QMHPA taught Chevelle problem-solving strategies to assist with decreasing her symptoms of depression. Chevelle appeared to be receptive to QMHPAs’ interventions and services that were provided to her during the session. Chevelle reported that calming techniques to relax when frustrated at situations, people, and things will support her with reducing her stress levels. Chevelle reported to having too much time to herself which she thinks a lot. She expressed that she struggles with racing thoughts that sometimes she can't control her on anxiety. Chevelle revealed that she journals but it has been inconsistent. Chevelle expressed to QMHPA that her level of self-understanding about self-defeating behaviors are linked to feelings of depression.
